• You must be an adult patient who has received services at UF Health.
• An activation code is typically provided during a visit to UF Health; however, you can sign up directly on the MyUFHealth login screen if you don't have one.
• You will need to provide some personal information to create your online account.
• A valid email address is required for MyUFHealth account creation.

Entering Your Secure Patient Portal

1. On a Computer: Navigate to the official MyUFHealth login page: https://mychart.shands.org/mychartprd/Authentication/Login.
2. Enter your MyUFHealth Username (which may also be your Epic ID) and your Password into the respective fields.
3. Click the Log in button to access your patient portal.
4. On a Mobile Device: Download the MyChart mobile application from your device's app store (developed by Epic Systems).
5. Once installed, open the app and search for and select UFHealth as your healthcare provider.
